Truman Health Services is a NCQA Patient Centered Medical Home which provides the highest level of care and latest treatment options for New Mexicans living with HIV, requiring gender services or seeking PrEP. We offer a variety of treatment services utilizing a holistic approach towards supporting our patients’ medical, social and mental health needs. Truman Health Services is seeking an experienced candidate to join our Case Management Team.

Truman Health Services is currently looking for an enthusiastic professional and experienced individual to assist people living with HIV maintain program eligibility and access social and medical resources. The Non-medical Case Manager will work as part of an interdisciplinary team at Truman Health Services by assisting people living with HIV to improve their lives by providing assistance in four areas: ensure patient has access to an HIV specialist, has access to HIV treatment and has stable access to food, shelter and transportation.

SUMMARY

Under general supervision, as part of a multi-disciplinary team, provides services relating to improving retention in care for specialized subsets of patients. Ensures eligibility for a variety of medical and social services and assists patients in completing appropriate paperwork. Non-medical case managers may provide assistance in accessing social, community, legal, financial and other needed services. May perform a range of administrative functions required to ensure efficient program operation.

DUTIES AND RESPONSIBILITIES

  1. Coordinates patient documentation to maintain eligibility for a variety of core medical and social support services;
  2. Conducts patient interviews required to complete intake applications and all required forms for core medical and social support services;
  3. Works closely with medical case managers to reduce patient barriers to obtaining needed medical care and /or social services;
  4. Assist with the identification of community resources that build support for medically and socially complex patients;
  5. Maintain current information on all regularly accessed community resources;
  6. Perform a range of administrative functions required to ensure efficient program operation.
  7. Maintain professional growth and development through completion of required departmental and New Mexico Department of Health competencies.
  8. Perform miscellaneous job-related duties as assigned.

MINIMUM JOB REQUIREMENTS

High School diploma or GED and 2 years of directly related experience. Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis. Verification of education and licensure will be required if selected for hire.

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED

  • Strong interpersonal skills and the ability and desire to work effectively with a wide range of constituencies in a diverse community.

  • Ability to effectively communicate information in a manner easily understood by the client.

  • Ability to gather data, compile information, and prepare reports.

  • Excellent organizational skills with the ability to prioritize duties.

  • Knowledge of community health care and vocational services and resources.

CONDITIONS OF EMPLOYMENT

  • NM driver’s license and ability to drive using own vehicle for community visits with mileage reimbursed at standard rates.

  • Must be employment eligible as verified by the U.S. Dept. of Health and Human Services Office of Inspector General (OIG) and the Government Services Administration (GSA).

  • Must pass a pre-employment criminal background check.

  • Fingerprinting, and subsequent clearance, is required.

  • Must provide proof of varicella & MMR immunity or obtain vaccinations within 90 days of employment.

  • Must obtain annual influenza vaccination.

  • If this position is assigned to a clinical area, successful candidate will be required to complete a pre-placement medical evaluation/health screen.  Required N-95 mask fitting, testing, vaccinations to include annual TST, Tdap, and Hepatitis B will be determined based on location and nature of position.

WORKING CONDITIONS AND PHYSICAL EFFORT

  • Work is normally performed in a typical interior/office work environment.

  • No or very limited exposure to physical risk.

  • No or very limited physical effort required.

  • Home, hospital & nursing home visits may be required.
