Other information:
Must hold a current Nursing license from State of New Mexico or active Social Worker License. Bachelor's Degree or 15 years of relevant experience preferred. Certification in an approved Case Management Program preferred. Five years of relevant acute care clinical experience required. Relevant management experience preferred. Three years of Case Management or Utilization Management experience, in a relevant setting.

Summary:
In collaboration with Clinical Operations Leadership, provides leadership for a Care Management department and oversees the departments daily activities. Coordinates the integration of Care Management functions to provide effective outcome based patient centered care. Serves as a resource to Care Management Leadership by developing tools, processes and systems to optimize the departments strategic goals. Provides support to staff, assigns duties to staff members based on volumes, skills, departmental needs. Identifies and mitigates problems as they occur and addresses performance issues. Orients, coaches, mentors the staff to meet the needs of patients of all age groups
