POSITION SUMMARY:
A Registered Nurse who is responsible for the direct clinical oversight of the provision of care as well as supervise clinical team. Responsible for planning, organizing, coordination and evaluation of all day-to-day aspects of Hospice patient care. This includes coordination of referrals and patient care related supervisory functions. JOB DUTIES 1.Plans, develops, directs, organizes, implements, and evaluates clinical services and activities. 2.Leads and participates as an active member in the IDG meetings. 3.Ensures that efficient systems, procedures, and controls are established for all clinical operations. 4.Works closely with the Executive Director-Hospice to plan and implement policies and procedures relative to clinical operations and set standards for all performance objectives. 5.Assures that adequate staffing levels are anticipated and maintained. 6.Participates in the identification of the need for, and assists in the coordination of, the development of new hospice programs and services. 7.Assures effective coordination between clinical staff and clerical staff so that paperwork necessary for payroll and billing is submitted accurately and according to established time frames. 8.Responds to all inquiries and referrals to the Hospice. 9.Determines staff assignments and assigns Hospice patients to the appropriate staff. 10.Coordinates patient care by all service disciplines providing care. 11.Assists with patient care as necessary. 12.Functions as a liaison between all disciplines providing care. 13.Provides direct supervision of the staff nurse providing the hospice care and assists the staff nurse in carrying out the role of the case manager. 14.Collaborates with inpatient staff to assure the implementation of the Interdisciplinary Plan of Care during inpatient stays. 15.Assures the timely completion of appropriate patient/family information in the on-call communication log. 16.May convene patient care conferences to assure effective provision and coordination of care. 17.Makes supervisory visits with patient caregivers as appropriate. 18.Promotes the utilization of all available resources for the benefit of patients, families, and team members. 19.Educates clinical personnel regarding Hospice admission criteria, policies and procedures, patient care and Hospice philosophy, and other related in-services. 20.Promotes public awareness of the services provided by the Hospice and the means to access these services through community education and/or marketing activities. 21.Is responsible for the Hospice's creation, maintenance and protection of accurate and compliant clinical records. 22.Is responsible for the implementation of the Hospice's professional and clinical services quality assurance program. 23.Participates in service related activities within the organization and the community. 24.Identifies need for staff and inpatient staff education and assists in the development of in-service programs for the Interdisciplinary Group. 25.Assumes responsibility for the Executive Director-Hospice during any absence. 26.Other duties as assigned by the Executive Director-Hospice. Requirements:
ESSENTIAL JOB QUALIFICATIONS: Education: Graduate from an NLN accredited school of nursing; BSN preferred. Experience: Minimum 2-years nursing experience in hospice or homecare, one-year supervision, education or nursing administrative service. Licensure/Certification: Registered nurse currently licensed in the State of New Mexico. Current CPR Certification. Current Auto Insurance. Valid Driver?s License Other Qualifications: Requires strong skills in the area of hospice operations, terminal care, knowledge of state and federal hospice care rules and regulations, and management team development. Effective communication and delegation skills are required. Must accept responsibility for maintaining clinical practice skills appropriate for the direction and supervision of Hospice's services, learning and implementing the policies and procedures of the Hospice on an on-going basis. Attendance: Regular attendance is a requirement of this position. Language: Must be able to read, write and speak the English language. Bilingual in Spanish preferred by not required.
