Shift: Days

01. Works with multidisciplinary team members to develop a plan of care for assigned patients.
02. Ensures patient is progressing towards desired outcomes by monitoring care through assessments, evaluations, and/or patient records.
03. Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
04. Performs testing procedures; reports and interprets test results.
05. Performs quality control testing; instrument maintenance and troubleshooting.
06. Identifies and resolves issues affecting the delivery of patient care.
07. Manages drug therapy protocols as a part of physician-pharmacist-nurse team.
08. Provides drug information and education to medical staff and patients.
09. Documents intervention and its outcome for use in drug therapy quality improvement efforts.

Doctorate degree in Pharmacy (Pharm.D.) or Registered Pharmacist (RPh). New Mexico Board of Pharmacy License required. Less than one year experience in pharmacy practice. Has acquired the necessary practice skills in order to appropriately evaluate medication orders. Skills acquired through experience or self-study. Pharmacist clinician license preferred.
