* The Transplant Coordinator is a patient advocate and transplant team liaison and is responsible for appropriately utilizing any and all resources to achieve optimal patient outcomes.

* The Transplant Coordinator coordinates all aspects of pre/post transplant care with patients, families, nursing units, ancillary services, organ procurement agencies, donor hospitals, and all members of the transplant team.

* The Transplant Coordinator assesses the need for patient/significant other teaching and ensures the appropriate level of teaching occurs to maximize the patient's potential for health and self care.

* The Transplant Coordinator along with members of Transplant Team assess and evaluate physical, psychological, financial, and psycosocial needs of the transplant patient and family and work together to determine appropriate interventions to lead to the best overall outcome.

* The Transplant Coordinator acts as expert resource and educator for internal and external nursing staff on all aspects of transplantation on a state wide/regional basis.

* The Transplant Coordinator is responsible for the collection and reporting of all Medicare, UNOS, and research data as it relates to transplantation at Presbyterian Hospital; and assures compliance with all appropriate agency standards.

* Performs other functions as required.

Two to three years college level courses. Associate Degree/Diploma in Nursing. State of New Mexico Nursing License required. Three to five years of transplant or critical care experience required. Must be able to demonstrate the skills to provide care appropriate to the age of the patients served.
