Requisition Number: 28598 Location: NM - Albuquerque Type of Opportunity: Regular Full-Time FLSA Classification: Non-Exempt Minimum Experience: 3 years Minimum Education: Bachelors Degree Shift: Days * Facilitates the initiation and completion of the admission process to include the admission database, initiating the medication reconciliation process, documenting patient education provided on admission and completing patient valuable form. * Documentation of externally reported clinical data pertinent to disease process or patient history in medical record, completes checklist and returns to nurse manager. * Initiates physician admission orders and administers medications ordered for administration upon admission to the hospital. * Performs influenza and pneumococcal vaccination screening and ensuring vaccinations are ordered for the following day administration if appropriate. * Communicates patient care needs and status to RN assuming care of the patient once admission process is complete to ensure safe handoff of patient care. * Assists with patient discharges or transfers that enhance or compliment patient flow and throughput. Duties may include discontinuing intravenous access as ordered, entering discharge instructions into the clinical information and reviewing with patient, or any activity identified by the RN caring for the patient that expedites discharges or admissions. * Initiates and completes the utilization management process on night shift to ensure patients are admitted to the appropriate admission status and level of care. Also ensures the physician admission status order matches the designation in the ADT system known as Medipac. Utilizes InterQual software and criteria to assist with determination. * Works in close collaboration with Bed Management personnel and the house supervisors to ensure patient throughput and flow issues are addressed and minimized as appropriate. * Escalates patient throughput issues or conflicts to house supervisors, takes direction from house supervisors and Bed Management coordinators directing patient flow. * Notifies Bed Management Coordinator of all observation status admission orders to ensure Medipac entry matches physician admission order. * Obtains ACLS certification within first six months of employment to Access Care Team. Skills / Requirements
Associate's Degree, Diploma or Bachelor's of Science in Nursing. State of New Mexico registered nursing license and two years of recent experience in a acute care facility. Advanced Cardiac Life Support (ACLS) within first six months of employment
