Requisition Number: 28591 Location: NM - Albuquerque Type of Opportunity: Regular Full-Time FLSA Classification: EXEMPT Minimum Experience: 1 year Minimum Education: Masters Degree Shift: Varied Primary Functions Uses an appropriate problem-solving approach to plan service. a. Performs complete medical history and physical exam on patients. Establishes differential diagnosis. b. Orders appropriate lab and diagnostic procedures. c. Synthesizes data to determine primary diagnosis and therapeutic plan utilizing established medical principles. d. Demonstrates the ability to recognize and critically assess any changes in patient's condition and reports such to the hospitalist physician. e. Coordinates acquisition of outpatient records from primary care provider or referring provider. f. Acts as a liaison and facilitator between on call physician, staff members, case managers, and attending physician regarding patient. g. Rounds with and without on call physician, ensuring all lab work, diagnostic results and other test results are available on chart and notifies the physician as appropriate. h. Facilitates coordination and development of a comprehensive and individual plan of care in collaboration with patient, family and multidisciplinary team. i. Acts as a hospitalist service liaison with patient care staff. j. Facilitates one on one communication with primary care physician regarding current and ongoing patient care needs. 2. Provides services with consideration of the recipient's needs. a. Interprets and integrates patient data to determine appropriate diagnostic and therapeutic procedures needed. b. Prepares written prescription orders, medications, and controlled substances under the rules and regulations of the State of New Mexico and the Federal DEA. c. Applies approved criteria to monitor appropriateness of hospital admissions and continued stays. d. Monitors implementation of guidelines for appropriateness of resources, trends and tracks guideline variances. e. Triages patient/family calls after discharge and up to first outpatient follow-up visit. f. Provides follow-up with patient/family after discharge in conjunction with team member activity. g. Identifies appropriate venue for follow-up care within the continuum. c. Coordinates patient and family education with nursing staff ancillary staff and other resources. d. Communicates with patients/families to ensure understanding of discharge instructions. e. Formulates, implements and evaluates strategies for specialized education as it relates to patient care. f. Utilizes critical thinking and problem solving techniques. g. Is able to work independently, exercising sound judgement, h. discretion, and initiative to facilitate change. Uses equipment/supplies correctly. a. Follows established procedures for computer and PDA. b. Follows manufacturer's guidelines for all equipment usage. 4. Uses appropriate safety and infection control measures. a. Corrects situations that pose a threat to patient or staff safety. 6. Reports/records information appropriately. a. Dictates patient information in appropriate format and following established guidelines. b. Maintains daily progress notes using established guidelines. * Performs other functions as required. Skills / Requirements
Master's degree in nursing. New Mexico license required. One to three years experience in family practice.
