POSITION SUMMARY:
A non-professional, who under the direct supervision of a Registered Nurse or therapist, as appropriate, provides medically necessary assistance with personal care in accordance with physician order. The Home Health Aide is supervised by a registered nurse or therapist every two weeks, either when the Home Health Aide is present to observe and assist, or when the aide is absent, to assess relationships and determine whether goals are being met. JOB DUTIES 1. Follows current written care plans as established by supervisor. 2. Provides personal care such as bath, hair care, oral hygiene, skin care, hand care and foot care. 3. Assists with bed and commode transfers and ambulation both with and without equipment. 4. Helps with prescribed exercises taught by the appropriate professional personnel. 5. Assists with self-administered, non-prescription and prescription medications. May not pour medication, but may hand patient medication bottle or prepared medications under the supervision of a registered nurse. 6. Takes and records vital signs. 7. Reinforces instruction of the patient and caregivers in good hygiene and personal care requirements. 8. Observes the patient and reports subjective and objective changes to the appropriate professional personnel. 9. Keeps an accurate record of each patient visit. 10. Participates in agency in-services and team conferences. 11. Completes timely and accurate documentation including time cards and all required clinical communication forms. 12. For Private Services Division: Upon hire must complete timely and accurate documentation of 40 hours of initial continuing in-service education their first year. Completes and provides documentation of 12 hours of continuing in-service education each year thereafter. 13. For Intermittent Division: Completes and provides documentation of 12 hours of continuing in-service education each year. 14. Accepts additional responsibilities as deemed appropriate by the Supervisor 15. Provides nutritional support Skills / Requirements
Requirements:
