Job: Client Support Assistant (0803561)

Position Summary: Term appointment; funding available through 6/30/2010; continuance beyond that date is subject to availability of additional funding.

There are two Full time (1.0 FTE) positions available.

Travel to local communities and member locations; this position in Albuquerque is office-based.

The Community Access Resources and Education Program (CARE NM), in the Department of Family and Community Medicine at the UNM School of Medicine, seeks to employ a motivated individual who can respond to the daily challenges and multiple duties of this program and its mission statement: "Empowering individuals for a healthier tomorrow."

The CARE NM Program provides personalized field case management services to individuals needing assistance with complex issues of daily life, including: personal and family adjustments, finances, employment, housing, food, and assistance with navigating the use of medical care services. The successful candidates should have a high level of skill, organization, initiative, be a self-starter and a team member. The individual for this position should possess strong communication skills, both oral and written, and who will work in a field setting, including client's homes. The applicant should also possess the ability to network with other agencies, coordinate services and prepare and present monthly member reports.

Minimum Qualifications:
High school diploma or GED; at least 3 years of experience that is directly related to the duties and responsibilities specified.

Preferred Qualifications: Bilingual in English/Spanish

Experience working with/in:

Community setting, working with diverse populations (including culturally, socioeconomically, educationally, and all ages).

Interviewing clients having personal and family problems, help with adjustments, finances, employment, food and housing.

Coordinating activities involving community resources on behalf of the client(s) by creating a plan and checking to make sure support services are being provided.

Monitoring and tracking of support services.

Other social, community agencies and health care providers.

A formal assessment process.

Clinical and field setting.

Proficient with:

Client Support Software regarding patient information, or similar program.

Microsoft Office Suite (to include MS Excel, Access, and Word).
